Malignant fibrous histiocytoma (MFH) rarely originates from the chest wall region. MFH has a high rate of local recurrance and distant metastasis. We present a case of MFH arising from subcutaneous soft tissues of the right scapular area. En-block resection of the tumor with the overlying skin was performed and the frozen section examination of the resection specimen was reported that the tumor was possibly a sarcoma with safe resection margins. The defect was reconstructed with a skin graft harvested from the lateral region of the right tigh. The final histopathological examination revealed that the tumor was a pleomorphic type malign fibrous histiocytoma, and measured as 18x14x7cm in diameters with negative surgical margins. Adjuvant radiotherapy was applied. The patient is alive without recurrence five months after the operation. In this study, the case is persented for it rarely originates from the chest wall area and the diameters of the tumor.
