A CASE OF LEGIONELLA PNEUMONIA WITH SERIOUS CLINICAL PROCESS

Legionella pneumophila is a cause of atypical pneumonia which has a serious clinic process and high mortality. FortyÐone years old female patient attempted to emergency department with these symtomps; weakness continued 3 days, fever, cough, sputum and dyspnea. The patient who had been followed because of pulmonary hypertension had a coronary intensive care unit stay history for vasoreactivity test. In physical examination; fever (39ºC), tachypnea (30/min), hypotension (90/60 mHg) and syanosis were present. In chest X-ray bilateral consolidation that is more mareable on left side was found and in thorax CT (thomography) bilateral patched consolidation and frosted glass scene were observed. Piperacillin-tazobactam and claritromisin were applied to the patient who was interned to intensive care unit for respiratory failiure. After L. pneumophila was found in sputum by DFA examination, rifampycine is added to therapy. Although treated with mechanical ventilation patient died on second day of therapy. We aim to emphasize L. pneumophila (as distinct from expected) has a high mortality in spite of appropriate antibiotherapy and treatment with invasive mechanical ventilation in intesive care unit.
