Congenital lobar emphysema(CLE) is a congenital abnormality with an unknown ethiology characterized by hyperinflation of lower respiratory tract. It is an unfrequent rare clinical condition, often recognised during early childhood. We aimed to present a rare adulthood presentation.

A 28-year-old male patient presented with back pain and dyspnea. Further clinical investigation was undertaken due to localised hyperinflation in left lower lobe. Dynamic computed tomography results were consistent with CLE and bronchial atresia. The patient underwent thoracoscopic wedge resection. CLA is a rare clinical condition, diagnosis during adult ages is even rarer.
